To submit an individual Blue Cross online applications click here- you will be redirected to the Blue Cross website to sign up for the application process.

Instructions to apply for individual or family coverage:

After entering Blue Cross website through the link above, click on the left side menu on +Individuals and Families
Click on Apply On-Line Now
Create a profile with user name and password and continue through the application process completing all fields as you go along. You can save it and return to log in again to complete at a later time if desired.
The initial premium payment can be made either with a credit/debit card charge; set up an automatic monthly checking account draft; or mail in a check directly to Blue Cross within 10 days - you will not be billed for the initial payment. Address and detailed instructions are provided towards the end of the application process.
All future premium payments can be either monthly automatic credit/debit card charge; automatic monthly checking account deduction; bi-monthly or quarterly direct bills.
