After entering the Kaiser website through the link above, you can put in your home zip code & age and get a quote.

Create a profile with user id and password & register at this site.

VERY IMPORTANT - in order for Ken Stamey and his office to assist you in any matters regarding the application process or after approval & claims issues, his name and broker information must be included in the proper fields in the online application. At the registration process, Step 1, there are 2 fields to include his Broker name and Broker ID #. Please complete the 2 fields as follows:

There are 3 easy payment methods to choose from. Kaiser will send out a bill after approval. You can pay by personal check or money order with each monthly billing. Or pay using EFT - which will have payments taken directly out of your checking account. The EFT option can be set up using a special form.
Kaiser will notify you by email within 5-7 days regarding the status of your application.
